Catalyst Kinetics Group offers various ways to book an initial assessment with one of our practitioners: Online, Jane App app, Email or phone call. All new clients are required to book an initial assessment before booking any other services.
Create Your Account
Catalyst Kinetics Group clients must set up an account profile in order to book an
appointment, use our online booking platform, or book and appointment using the Jane App. You can create an account with Catalyst Kinetics by completing the CKG Intake Web Form and clicking on the “Submit Form” button at the bottom of Page 3.

Book Your Appointment
Catalyst Kinetics Group offers various ways to book an appointment with your health care providers: Online, Jane App, email or phone call.
If you are a New Patient, you will be required to book a New Patient Assessment. If you are an existing patient and have not been to Catalyst Kinetics Group within the last year, you will be required to book a New Patient or Readmission appointment depending on the department. In either case, we want to ensure your health history is up to date and that your practitioner can review any changes that have occurred within that year. It is advisable that all patients who are returning to Catalyst Kinetics after a year proceed to Step 2 and complete the first four (4) pages of the Catalyst Kinetics Health Questionnaire.
Complete Health Questionnaire
After you have created your account, you should download the Catalyst Kinetics Group Health Questionnaire and complete it in preparation for your New Patient Appointment. Client who have not attended an appointment in over a year should also complete the first four (4) pages of the Health Questionnaire in preparation for their Reassessment Appointment.
We are aware of the time it takes to complete a comprehensive health history, however, your cooperation in completing this information is essential to providing the highest standard of care. Please complete the Health Questionnaire to the best of your ability. We encourage all new clients to complete the entire Health Questionnaire, however, if you are in a rush, please complete the pages indicated according to the practitioner(s) you have booked appointments with.
Please email the completed form to reception a few days before your appointment. If you are unable to email the form please bring the completed hard copy with you to your appointment.
On the day of your appointment
Arrival Time: Please arrive at least 5 minutes before the start time of your first appointment and bring your completed Health Questionnaire if you are a New Patient and did not email it to reception prior to your appointment. If you were not able to complete the Health Questionnaire, please arrive at least 15 minutes before your appointment to complete the form in our clinic.
Parking: Access the Catalyst Kinetics parking lot off of 4th avenue and 175th St. Parking is available in the complex and we have reserved spaces on the side of the building. The entrance for the clinic will be noticeable across from Bentley Dog Groomers, on the south side of the building
Clothing: Please dress comfortably so that we can more accurately observe your problem areas and assess your movements. Shorts are recommended for lower extremity injuries, and a loose fitting T-shirt or tank top is best for upper body and neck injuries. Your clothing should be comfortable and fit in a way that does not restrict your movement so you can still bend, squat, or lift your arms above your head.
Mask/PPE: Some practitioners may be wearing PPE and request their patients to do the same. If a patient would like their practitioner to wear a mask, they are more than happy to oblige your request.
Credit Card: Please ensure you have added a valid credit card to your Catalyst Kinetics JaneApp account. You may access your account at the following link.
