Billing Options

DIRECT BILLING FOR THIRD PARTY INSURANCE

To have your treatments billed directly to your insurance company, we require the following by patients:

  • *Health insurance provider information must be completed online by the patient by requesting the “updated account & clinic policies form” from our administration, a scanned copy of your insurance card uploaded to your account and a valid credit card added to your account for any user fees or rejected or maxed claim payments.
  • *We direct-bill to the following: Please review the image below
  • *We only direct-bill the primary insurer and will not direct-bill under a secondary insurer (spouse plan) even it is still with any of the insurance providers below

*We will NOT process direct billing requests without a credit card on account for any patients.
*We are unable to check how much your insurance plan will cover.

Direct Billing

THIRD PARTY INSURANCE PROVIDERS

ICBC DIRECT BILLING

To have your treatments direct billed to ICBC, we require the following by patients:

  • *Date of MVA is within 12 weeks of initial appointment – early access treatment period. Patients are automatically eligible for certain treatments. Please view the ICBC link for further information: https://www.icbc.com/partners/health-services/Pages/ICBC-changes-to-insurance.aspx
  • *Motor Vehicle Accident form complete online and signed third party billing agreement
  • *Add a valid credit card to your account for any user fees and for any rejected treatment invoices by ICBC (If treatments occur past the approved discharge date and/or if you have gone over the maximum approved number of treatments). We will NOT approve direct billing without this.

Patients are responsible to keep track of the number of treatments if they are approved or and the expiry date. If you require a treatment plan to be submitted to ICBC please speak with your practitioner prior to the expiry date. If the approved treatments have expired patients will be privately charged for that session as we are unable to direct bill it to ICBC.

**If your MVA is outside the 12 weeks of approval, please provide our MVA Coordinator with an emailed copy of documented approval from ICBC for direct billing of treatment prior to booking an appointment. Email: MVA@catalystkinetics.com

***Failure to provide approval or the above requirements being completed will result in private payment for the appointment by the patient on the day of treatment.

MEDAVIE BLUE CROSS (RCMP/VETERANS AFFAIRS/CANADIAN ARMED FORCES)

To have your treatments direct billed to Medavie BlueCross, we require the following by patients:

  • *Medavie Blue Cross card upload to your profile or physical copy brought to your initial assessment
  • *GP referral for Naturopathic and Massage Therapy treatments
  • *No Naturopathic or Kinesiology treatment coverage for Veterans Affairs
  • *Add a valid credit card to your account for any user fees and rejected or maxed claim payments. We will NOT approve direct billing without this.

WORKSAFEBC (CHIROPRACTIC CLIENTS ONLY)

To have your chiropractic treatments direct billed to WorkSafeBC, we require the following by patients:

  • *Valid WorkSafeBC claim number or for recent WSBC accidents if your claim number is pending we can still accept your enrollment
  • *Date of Work Injury/Accident must be within 8 weeks of initial appointments
  • *If you have seen a Chiropractor at another clinic for your WSBC claim we are unable to direct- bill to WorkSafeBC under the same claim number
  • *MSP (Personal Health Number required)
  • *Injury Accident form complete online and signed third party billing agreement
  • *Add a valid credit card to your account for any user fees and rejected or maxed claim payments. We will NOT approve direct billing without this.

MEDICAL SERVICES PLAN (MSP)

Our clinic does NOT direct-bill to Health Insurance BC under your MSP. Patients are required to make payments on the day of treatment and will be provided a receipt.

*We do not adjust the appointment cost for MSP.

We accept payment by Debit, American Express, MasterCard, Visa, and Cash. Payment is required in full on the day of treatment.

1st Late Cancellation (LC) or No Show: LC Fees will be waived = D100%

2nd Late Cancellation (LC) or No Show: LC Fees = 50% Price of Scheduled Appointment

3rd and Subsequent Late Cancellation (LC) or No Show: LC Fees = 100% Price of Scheduled Appointment
